  • Complete the nomination form
    You will be asked to enter your operational revenue for the years 2005, 2006, 2007, 2008 and 2009.
    Please learn the amount before you start to fulfill the form, otherwise it may take a while and you may be logged out.
  • Submit Annual and/or Financial Reports for 2005 and 2009
  • Submit brief description of company and proprietary technology (maximum of 300 words)*
  • Color portrait of CEO in JPG format*
